Admissions Process

Families come to Brookside’s Office to get registration materials.

• Registration form
• Release of Records   [New students]
• Notarized Affidavit for Admission   [New Students]

Return completed registration materials to Brookside’s Office.

• Returning families – April (Current families who have not turned in their registration forms risk losing their children’s spaces in the school as Open Registration begins.)
• Open Registration begins is also in April

Based upon availability of class space, families will be notified to pick up the following enrollment materials at Brookside’s Office:

• Enrollment form (needs notarization)
• Health form
• Meal Benefit form
• Parent/Guardian and Student Contract

After Brookside sends enrollment materials, families have two (2) weeks to return completed enrollment materials to Brookside’s Office. Families who fail to meet this deadline will lose their children’s spaces in the school, and their children will be placed on the waiting list. Children are not admitted to Brookside for 2007-2008 until Step #3 – ENROLLMENT is complete.

Brookside will send written confirmation to the families whose children have been admitted to Brookside for 2007-2008.
